For healthy adults, this means a miserable week or two. You'll be contagious and feel wretched, but you'll recover at home with rest and fluids.

Older adults — particularly those over 75 — show hospitalization rates approaching what we see in young infants. The contagious timeline:
Most adults become contagious within a day or two after exposure, often before they feel sick. This pre-symptomatic spread makes RSV maddeningly difficult to contain. You're out in the world feeling fine, shedding virus with every conversation.
Peak contagiousness hits during the first three to eight days of symptoms. Your nose is streaming, you're coughing constantly, and each cough aerosolizes thousands of viral particles. If you're going to infect your household, it happens during this window.
Here's what catches people off guard: you can remain contagious for one to two weeks total, sometimes longer. That lingering cough? Still potentially infectious. Healthy adults usually clear the virus within a week, but immunocompromised individuals can shed virus for three to four weeks or more.
Why this contagiousness pattern creates real problems:
The overlap with holiday season — RSV peaks November through March in most of the U.S. — means maximum viral spread exactly when families gather. One infected person at Thanksgiving can seed infections across multiple households within days.
Healthcare settings see nosocomial RSV transmission regularly. An infected patient in a shared room, a healthcare worker working through "just a cold," visitors bringing the virus in — it spreads rapidly through vulnerable populations.
Workplaces face the same issue. Open office plans, shared conference rooms, communal break rooms — RSV moves efficiently through these spaces. The "work through it" culture means people show up sick, extending chains of transmission.
Practical infection control that actually works:
Isolation during peak symptoms isn't optional if you're around high-risk individuals. That means staying home from work, skipping the family gathering, avoiding your immunocompromised friend. The virus doesn't care about your schedule.
Hand hygiene matters more than most people practice it. Soap and water for 20 seconds, particularly after coughing, before eating, after touching shared surfaces. Hand sanitizer works but isn't quite as effective against RSV as thorough washing.
Surface disinfection helps in shared spaces. Regularly wiping down frequently-touched surfaces — light switches, faucets, remote controls — reduces transmission within households. The virus doesn't survive indefinitely on surfaces, but it persists long enough to infect the next person who touches that contaminated doorknob.
Masks work, though few adults choose to wear them for a bad cold. If you must be around vulnerable individuals while sick, a well-fitted mask significantly reduces transmission risk.
The household transmission question:
If someone in your home has RSV, assume it will spread. Studies of household transmission show secondary infection rates of 45% or higher. You can reduce risk with aggressive hygiene and distancing, but eliminating it entirely is nearly impossible when sharing bathrooms, kitchens, and air space.
The decision point becomes: who in your household is high-risk? If everyone's young and healthy, household spread is inconvenient but not dangerous. If you're caring for an elderly parent or an immunocompromised family member, aggressive isolation within the home becomes critical.
Prevention and What to Actually Do About Symptoms
The vaccine landscape just changed:
For decades, no RSV vaccine existed. That changed in 2023 when the FDA approved two vaccines for adults 60 and older: GSK's Arexvy and Pfizer's Abrysvo. Clinical trials showed both reduce severe RSV disease by about 80% or more in this age group.
These aren't universal recommendations yet. The CDC advises shared clinical decision-making — meaning you and your doctor discuss whether vaccination makes sense based on your specific risk factors. Someone with COPD or heart failure should strongly consider it. A healthy 62-year-old marathon runner might reasonably skip it.
A third product, the monoclonal antibody nirsevimab (Beyfortus), protects infants but isn't approved for adults.
If you fall into the 60-and-older category with chronic conditions, asking your doctor about RSV vaccination is worth the conversation. The vaccines are new enough that long-term data is still accumulating, but the initial safety profile looks solid.
When you suspect RSV:
Home care suffices for most healthy adults. Rest, fluids, acetaminophen or ibuprofen for fever, saline nasal rinses for congestion. The virus runs its course in one to two weeks regardless of treatment. No antiviral medication reliably shortens duration in otherwise healthy adults.
Watch your breathing. Rapid respiratory rate — more than 20 breaths per minute at rest — suggests your lungs are struggling. Shortness of breath doing simple activities like walking to the bathroom is concerning. Wheezing or chest tightness needs evaluation.
Persistent high fever — above 103°F for more than three days, or fever that improves then worsens again — can signal secondary bacterial infection or other complications.
When to seek care immediately:
Severe shortness of breath or inability to catch your breath warrants emergency evaluation. If you feel like you can't get enough air or you're working hard to breathe at rest, don't wait.
Confusion or altered mental status can indicate low oxygen levels affecting brain function. If you or someone you're caring for seems disoriented, excessively sleepy, or not making sense, seek emergency care.
Bluish lips or face signals critically low oxygen and demands immediate emergency response.
Chest pain, particularly if severe or accompanied by shortness of breath, needs urgent evaluation to rule out cardiac complications.

The frustrating treatment reality:
No magic bullet exists for RSV in adults. The antiviral ribavirin, sometimes used in severely immunocompromised patients, hasn't shown clear benefit in immunocompetent adults and carries significant side effects and costs. Supplemental oxygen for low oxygen levels, IV fluids for dehydration, respiratory support for respiratory failure — these are supportive measures that buy time for your immune system to clear the virus.

What This Means Going Forward
RSV isn't new, but our recognition of its impact on adults is relatively recent. For years we undercounted adult RSV cases because we weren't looking for them systematically. Better diagnostic testing and increased clinical awareness revealed what was there all along: a substantial burden of severe respiratory disease in adults.
Climate patterns may be shifting RSV seasonality. Traditionally predictable November-to-March peaks are becoming less reliable in some regions, with cases appearing in unexpected months. This makes year-round vigilance more necessary.
The introduction of vaccines marks a turning point. We finally have tools beyond symptom management. Whether these vaccines become routine recommendations for all older adults or remain targeted to high-risk groups will depend on real-world effectiveness data and cost-benefit analyses currently underway.
For now, educated awareness serves you best. Know that RSV isn't just a pediatric problem. Recognize severe symptoms early. Protect vulnerable people in your circle through basic infection control.
